Brothers and Sisters,

The Executive Committee and the WSIB Committee are recommending that all Members who attended the Lottridge Street Fire June 2nd-4th do the following:

1. Fill out a WSIB form stating that you had exposure to byproducts of combustion from computer terminals as well as shredded poly vinyl chloride and other unknown products.

1. Fill out a Local 288 Exposure Form in the same manner and send to Ed Stanisz at Station 10D.

It is important that these forms be completed for future reference should you suffer some ill effects down the road.
